Applicant details
We'll ask for your first and last name, date of birth, and gender. We also inquire about your income, whether you are handicapped or disabled, and if you have a stable income or are currently employed. This helps us understand your basic needs and eligibility.
About your needs
We need to understand your current situation: your specific needs, how many people are in your family, and your monthly income. We also ask about any mental diagnoses, your race, if you have a case worker, who referred you to us, and how you heard about Journey Connections Independent Living.
